Replace lost, stolen, or damaged documents in Canada

Birth certificates, driver's licences, and health cards are all replaced through your province. SIN confirmations and passports go through the federal government. Pick your province for a free step-by-step checklist with official links.

If your documents were stolen

File a police report first and keep a copy. It's useful for the replacement application and for notifying your bank if you're concerned about identity theft.

SIN — Confirmation of SIN

Service Canada no longer issues SIN cards. Instead, you request a free Confirmation of SIN letter via My Service Canada Account or in person at any Service Canada centre. No fee.

Passport (lost or stolen)

A lost or stolen passport cannot be reordered — you must report it and apply fresh. Call Passport Canada (1-800-567-6868), then complete form PPTC 203. A $45 surcharge applies if the passport expired within the past 3 years.
