The extra $45 fee for a lost or stolen passport applies when the passport was still valid; you include a declaration (PPTC 203) with the application and pay the regular fee plus $45. If the lost passport had already expired, you renew or apply in the normal way, with no special form. We had tied the $45 to a passport that expired within the past 3 years.
$45 surcharge if the passport expired within the past 3 years → $45 replacement fee (with PPTC 203) when the lost or stolen passport was still valid; an expired one is renewed the normal way
